What is Otoplasty?

Published by Reza Nassab

Otoplasty, commonly referred to as ear surgery or pinnaplasty, is a cosmetic procedure aimed at improving the shape, position, or proportion of the ears. It can correct a defect in the ear structure that is present from birth, treat misshapen ears caused by injury, or bring the ears into better proportion with the face. This procedure is popular among patients seeking to address overly large ears or ears that stick out prominently.

How is Otoplasty Performed?

Otoplasty is typically performed under local anesthesia or sometimes under general anesthesia, depending on the complexity of the surgery and the patient’s preferences. The specific techniques used during otoplasty include the following steps:

  1. Incision: An incision is made in the back of the ear to access the cartilage. This runs along the back of the ear and is usually well hidden.
  2. Cartilage reshaping: The cartilage is bent toward the head. This recreates the antihelical fold of the ear. In some cases, excess cartilage of the conchal bowl may be removed or the bowl sutured to the mastoid area to bring the ear closer to the side of the head.
  3. Stitching: Permanent stitches may be used to help maintain the new shape of the ear. These stitches are carefully placed to anchor the ear until healing occurs.
  4. Closing the incision: The incisions are closed with stitches, which are dissolvable and will usually fall out after about 1-2 weeks.

The entire procedure usually takes about an hour, depending on the extent of the surgery.

Who is the Ideal Candidate for Otoplasty?

The ideal candidates for otoplasty are individuals who are in good health and have realistic expectations about the outcomes of the surgery. Specific criteria include:

  • Age: Children who are at least 5 years old, as their ear cartilage is stable enough for correction. Adults can undergo otoplasty at any age. We only offer otoplasty at our clinic for patients aged over 18 years.
  • Non-smokers: Smoking can complicate both the procedure and the healing process.
  • Individuals with protruding ears or large ears that affect self-confidence.
  • Patients without serious medical conditions that impair healing.

What is the Recovery Following Otoplasty Surgery?

Recovery from otoplasty is relatively quick and most people will be back at work within a few days. Here are common post-operative experiences and recommendations:

  • First 24 hours: Initially following surgery there will be minimal pain and discomfort since the local anaesthetic will last about 4-6 hours. This will gradually wear off resulting in some discomfort. This discomfort can be managed with simple painkiller such as paracetamol. There will be a headband on for the first week. You should sleep with your head elevated for the first 48 hours.
  • First week: The first week will involve wearing a headband during the day and night. This should not be too tight as that can cause problems with healing. The ears are usually glued to the side of the head for additional support. This glue also helps protect the incisions. You can shower and wash the hair after 3-4 days but must avoid rubbing the area around the ears. There will be bruising and swelling during the first 1-2 weeks.
  • 1-2 weeks: After the first week, you can take off the headband. The ears may seem very far back due to the glue keeping them stuck back. The glue will gradually come away over the next 1-2 weeks and the position of the ear come more forward which is normal. You will have an appointment to see us at 1-2 weeks to check the wounds. You should wear the headband at night for the next 5 weeks to help prevent any damage whilst sleeping.
  • 4 weeks: You can return to normal activities such as exercise or swimming.
  • After 6 weeks: You will have a follow up appointment to see us again.

Do I Need a Headband After Otoplasty Surgery?

Yes, wearing a headband after otoplasty is recommended. The headband helps to maintain the new position of the ears during the initial phase of healing and reduces tension on the sutures. It’s particularly crucial during the night to prevent the ears from bending or folding incorrectly as you sleep.
